For the discount-broker settlement, the official administrator is Verita Global and the official site is TrailingCommissionsSettlement.ca. If you held the funds somewhere other than through a discount broker, a separate settlement applies.
Multiple notices can be legitimate when details across CIBC/Renaissance accounts differ. If you received multiple Known Claimant notices with unique Claim IDs and PINs, the administrator says to submit each one separately.
